In the event of ticket loss, a new ticket must be issued, with the same exact trip as the ticket that was originally issued. Changes in names, dates or routes will not be permitted.

 

To issue a new ticket the following will be necessary:

 

1. Fill out and sign the Declaration of Lost Flight Ticket in legible writing (signature may not be digitalized); joined with a copy of an identification document with the same signature as in the declaration;

 

2. Send completed Declaration and 1 (one) copy of ID to:

3. Make payment of administrative fee (R$ 95,00 for tickets issued in Brazil and US$ 95,00 or the equivalent in solicitant’s local currency for tickets issued abroad), through the Smiles Service Center.

 

Terms & Conditions:

 

• The new issuance of the ticket will be made upon receipt of specified documentation in item 1 above and payment of the administrative fee.

 

• The ticket that was originally issued will be cancelled and use of original ticket will be liable for judicial measures taken by the relationship program.
